Next topic for discussion:  Dhatu Kshay #Ayurveda
Dr.Piyush Dodiwadiya: Ras dhatu kshay Symptom can be co relate with, decreased immunity... Shram glani shabd ashahisnuta
Ayulink: One basic question: how many of us practically examin Dhatu kshay? or dhatu vruddhi?
How to understand Ras dhatu kshay? is there any easy method for that?
Vd. Pulkit D. Patel: Sir in reality we are only focus on symptomatic treatment and its a main problem of ayurveda
**********************************************

Ayulink offers Arthrothermia treatment for any kind of Joint/Ligament/Muscular disease.

For more details visit http://ayulink.com/arthrothermia/

***********************************************

Dr.Piyush Dodiwadiya: Majja dhatu kshay Bone marrow depression. On CBC report we will find low wbc low hb low platelates. In any severe viral infection like dengue, cbc shows above finding And due to severe weakness pt feels bhram timirdarshan etc...
Ayulink: There is a very simple clinical method to check rasa dhatu. Hold your both hands raised from elbow to your waist height. Make light (not very tight) fists.  Now raise both hands on your head, keep them there for 10 seconds.  Bring both hands again in waist height, and open yr fist in blink. Observe both palms, changing colour. The micro second you see colour changes from pale to red. If rasa dhatu disturbance is there, the changing of color will take little more time. If rasa is good, you will take very less time - almost nil.  I think I have written in proper manner to guide you properly.
Dr. Rachana Mumbai: Will definitely check with patients for this
Dr.Piyush Dodiwadiya: Sir Is it capillary reffing or something like that
Dr. Pritam Veer:
 रस क्षये हृत् पीडा कम्प शून्यताः तृष्णा च॥(सु.सू.१५/)
रस (क्षीणे)रौक्ष्यं श्रमः शोषो ग्लानि शब्द असहिष्णुता॥(.हृ.सू.११/१७)
Rakta Dhatu Kshaya – depletion of Rakta (blood) – Blood tissue gets its nourishment from Rasa Dhatu. Rakta and Pitta are directly related. Increase / decrease of Pitta dosha directly causes the same with blood tissue. Depletion of blood tissue causes symptoms of anemia, which are explained as – Parusha – Roughness, Sphutita – cracks, Mlana- dullness, Tvak Rukshata – skin dryness.
Mamsa Dhatu Kshaya – depletion of muscle tissue – Muscle tissue gets its nutrition by Rakta Dhatu. Depletion of muscle tissue directly causes weakening of limbs and causes  – Sphik, Greeva Udara Shushkata – emaciation of the buttocks, neck and abdomen.
Medo Dhatu Kshaya – depletion of fat tissue – Fat tissue gets nourishment by Mamsa Dhatu. Depletion of this causes thinness of the body.  Sandhi sphutana – cracking of the joints, Glani – Lassitude, Akshno Ayasa – tired eyes, Udara tanutva – thinness of the abdomen.
Asthi Dhatu Kshaya – depletion of bone tissue – As per Ayurveda, bone tissue gets nourishment by fat tissue. This is why, oral administration of oil, ghee etc are advised during osteoporosis, osteo arthritis etc. Bone tissue is inversely related with Vata Dosha. This means that, increase of Vata dosha causes depletion of bone tissue and vice versa. Ayurveda explains that teeth, nail, mustache are also related with bone tissue. Hence, depletion of bone tissue exhibits the following symptoms. – Kesha, Loma, Nakha, Shmashru, Dvija prapatana – falling of hair, nails hair of the beard including mustaches and teeth, Shrama – tiredness, Sandhi shaithilya – looseness of joints.
Majja Dhatu Kshaya – depletion of bone marrow – As per Ayurveda, marrow gets its nourishment by Asthi dhatu.  depletion of Majja dhatu causes – Asthi Sheeryata – emptiness of bones, Durbala, Laghu Asthi – thinness, weakness, and lightness of the bones, Vata Roga – frequent affliction with Vata imbalance disorders.
Shukra Dhatu Kshaya – depletion of semen / female reproductive tissues – As per Ayurveda, Shukra dhatu comprises of male and female reproductive system. It gets its nourishment by Majja Dhatu. Its depletion causes – Daurbalya – weakness, Mukha Shosha – dryness of mouth, Pandutva – pallor, Sadana – lassitude, Shrama – tiredness, Klaibya – impotency and Shukra Avisarga – non-ejaculation of semen, non-ovulation.
Ayulink: Rasa vaha srotas refilling , I think. I have learnt this from my guru.
Vd Lakshmi G: What might be the reason for mamsa dhatu kshaya? Is there any reason like tulya gotra vivaha? If so... can anybody explain?
Dr.Piyush Dodiwadiya: Why you concentrate particularly on tuly gotr And what symptom you observe in mamsa dhatu kshaya. Protein deficiency can be consider . Muscle wasting is there in some neurological condition also like motor neuron disease
Ayulink: How to check Mamsa dhatu kshay? I can share with you an easy method. Ask the patient to show you palm and finger (back side)....make tight and close - all fingers. If you can see through between the fingers, is a mamsa kshaya vikruti....once again this is from my guru. If anybody does have other idea, pl share here. This photo froma young lady, age 23...very lean and slim (you can call her Vata person), Now here u can see the gap between 2 fingers. My Guru Late Vd. Kirit Pandya's interpretation was Mamsa dhatu kshay.  In aged people, in Vata people, in under nourished people  - you find it more.
The mamsa or muscle surrounding bones have decreassed at particular point.
Dr.Piyush Dodiwadiya: So Plan for management? According to symptom or any other specific rx
Ayulink:  saman guna vardhane.....or opoosite vata treatment...specifically abhyang, snehapan  etc.
I always ask  my students to check 7 dhatus before they decide any line of treatment. Majority of our practitioners make V-P-K diagnosis and thats all. But I personally feel that only VPK is not complete. The whole theory name is Dosha - Dhatu and Mala theory. Dhatus and Malas are also important to consider. May be I got this ffrom my Guru and that makes me successful in practice.
Vd Lakshmi G: Because the patient is balavan, no proper cause has been found, but it has been inferred that his father's is congenital marriage. So I want to know does the congenital marriage acts like causative factor for muscle dystrophy
Ayulink: *Clinical / Practical tips for Dhatu Examination*
Rasa - as I suggest earlier with lightly closed fist, raise above shoulder, bring it down rapidly after 30 sec, open the fist very fast and observe the change in palm colour
Rakta - Darshana, some questions too and later with modern lab tests (they are bio-chemistry, I dont consider modern labs as allopathic tools)
Mamsa - looking at mamsa surrounding bones
Meda - many parameters like BMR etc
Asthi - by looks - improper growth / decrease in bone joints. Particularly hand / leg fingers
Majja - Initially by questioning - later with the help of lab
Shukra - Initially by questioning - later with the help of lab
Dr.Piyush Dodiwadiya: Sir for majja what kind of lab
Ayulink: Bone marrow testing
Dr. Arvind Shahane: In Vegiterian Pt  Mamsa dhatu kshay what is option?  Samane saman vruddhi?  Please elobrate Sir
Vd. Sachin Kadlag: गोधूम (  wheat ) , माष after agni - deepan is good choice .  abhyanga plays important role too. for abhyanga महामाष तैल  can be used .  cow milk and cow ghee regular use.
Dr. Pritam Veer: For Mansa dhatu daily abhyang with mahamash , baladi , balashwagandhadi are very good option. Dourbalya is main symptom of mans dhatu kshyay. Esspecially for athlets we recommend daily abhyang. As it increases strength which they need. Also increases their capacity.
Dr Anzil Eramangalath: For Mamsa dathu kshaya we can go for Mamsa kizhi (pottali) ,we use it muscle wasting and all...., very beneficial
Vd. Sureshbhai Chavda: Mansa dhatu kshaya  given  rajyapan kwatha basti with aja mansa Also given in majja dhatu kshay in majja oil basti. Raktshay  rakt basti. Ashthikshaya tiktkhsir aushadhi and ghrut basti.
Dr. Pritam Veer: Asthi Dhatu Kshaya – depletion of bone tissue – As per Ayurveda, bone tissue gets nourishment by fat tissue.
For Asthi dhatu kshyay mainly shatavari , Ashwagandha, vidari , balantshepa used.
For majja ksyaya giddiness is main symptom along with giddiness we find sleeplessness or nidra nash.  Best remedy for majjakshyaya is dugdha nasya. 4 -4 drops milk nasya until giddiness stops. Or any other ghrut or teil nasya for nidranash.
For Shukra kshyay dugdha paan , mash Sevan are good remedies.
Dr. Jyoti Ashok: Kooshmanda in food or rasayana form is v. good.in vegetarians
Ayulink: Usually we treat dhatu kshya conditions, do we also treat dhatu vruddhi conditions? Which?
Dr. Pritam Veer: Yes Medo vrudhi , Asthi vrudhi ( calcaneal spur).  Mansa vruddhi.

Next topic of discussion : Ajirna (indigestion?) & it's management with #Ayurveda
Dr. Arvind Shahane: Four types of ajirn
1) amajirn - Kafaj
2) vidagdhajirna - Pitaj
3) vishtabadhajirna - Vataj
4) ras sheshajirn
Ayulink: How to differentiate between them?
********************************************

Ayulink offers fast and assured relief for Migraine.

We have 7 Franchise in Ahmedabad, PalanpurJunagadh, ValsadPatan, Mahudha and Pune for the management of Migraine.

********************************************

Shail H. Bhavsar: First treatment of ajirna is told to be langhan. Anyone heared about sadyo-vaman in ajirna???
Vd. Vipul Patil: Sadyo Vaman is indicated in acute ajiran,it's common if someone overeating than they self induce vomiting, and get relax But in jiran ajiran again think of vaman
Dr. Satish Panda: Incomplete metabolism due to weakened digestive fire leads to unprocessed state of food causing ajirna. ajirna due to agnimandya is the root cause of many diseases. If Ajirna is ignore it may lead to 8 uncurable diseases like vata vyadhi, ashmari, kushta, prameha, udara, arsha, grahani.
Gary Yuen: Is it not more accurate to say choosing food according to moment and adjusting quantity of ingredients according to properties such as heaviness.
Dr. Satish Panda: First line of treatment = langhana (fasting or lightening of intake) Treatment:  Causative factors: heavy meals and eating before the next meal digested, irregular diet (vishamashen) avoid causative factors langhan (if aam less, hot water, shunti water) jeeraksha: hot water w/soaked cumin seeds, settle for an hour, then sip. deepan pachan (aam pachan) lakshan: sometimes constipated/sometimes loose stools (best drug is haritaki – it’s deepan, pachan, and vata valoma and mild purgative. Use for constipation. If loose, kutaaj. (Good for dysentery.) Musta (has all 3 deepan, pachan, graha – bec it has lost function of graha, brings tone. Churna w/warm water ½ hour before food to hold food properly. Takra: brings kshaya rasa which has property to hold (also best source of lactobacillus) Can add cumin powder to takra or danyak (coriander powder) Black raisins (dry grapes) 10-20 alone or with hot milk as anuloman Raisins aid with verechan, don’t cause it. Pomegranate juice (sour) good for grahani; seeds (astringent) If patient not having burning or gastric distress, give wet ginger and honey (honey=madhur, kshay). Chitrak: synonymous with agni. black pepper (mix with takra) light food: ½ food, ¼ water, ¼ vayu (cooked properly, no raw food/ mung kichari or mung dal, laja
Vd. Dhruti Kagrana:
Four types of Ajirna main symptoms
Aamajirna :- Atiudgar ( excessive burping)
Vidagdhajirna:Udarvyatha (Abd.pain/discomfort)
Vistbadhajirna:- Angabhanga(Whole body ache)
Rasasheshajirna:- Jumbha(Yawing)
Vd. Pritam Bharat Veer: Recommended when dosha are Utklishta ,means ready to come out from GIT.
Gary Yuen: Caraka Su 25: eating once a day favorises the perfect absorption of food.
Ayulink: Only agnimandhya? Which are the causes of Ajirna?
Gary Yuen: Indeed. Advise of caraka Su 5: sali, sastika, yava, mung dal, amalaki, etc. Among modern foods, all common meats are likely heavy compared to wild forest and desert animals.
Dr. Satish Panda: Causes : Disobeying the dietary rules is the prime cause. suppression of urges. Irregular meals. Over eating. Sleeping during day time. Drinking excessive water unnecessarily. Not sleeping at night. Eating heavy food (i.e. food which are heavy to digest).Eating too much of spicy food. Adequate exercise also help to digest heavy food
Dr. V B. Maheshwari: Can anyone explain *aanshansh kalpana* in ajeerna vyadhi
Ayulink: Ajirne bhesajam vari means what?
Dr. Kalpesh Vani: In ajeernavastha vari ie. Koshna jal is the bheshajam ie. Medicine. No other medicine is required
Dr Jayesh Patel: Vari means enemy. Not to give medicines in ajirn. Cause in ajirn condition agni is not digest bheshaj (medicines).
Dr. Kalpesh Vani: Here in this shlok Vari means Jal not enemy. This is the shlok of Qualities of water
Vd. Sachin Kadlag: Yes, very important and has wild application regarding digestion. But here in ajeerna water is to be given with " Yukti " that is idea. like hot /warm water/ boiled water /half reduced water like different formulations of water according to different levels of angimandya / or according to type of indigestion (ajeerna ) can be used.
Vd. Hardik bhatt: Without any medicine only half boiled water has any chemical change??
Vd. Sachin Kadlag: more boiling more laghu
Vd. Dhruti Kagrana: May be bhaishja vari menas in Ajirna pt. Stay on liquid diet?
Vd. Pritam Bharat Veer: No liquid diet, only koshn JAL SOS. Ajirne Langhanam shrestham!
Dr. Vasupradha R: I guess वैरी s enemy
वारी s water. There's also a जलाजीर्णम्  nah.. In that too ushna jalam can be used?
Shail H. Bhavsar: After langhan And sukhoshna jala use Deepan pachan should be done. Some medicines are Chitrakadi vati, Aam pachan vati
Haritaki
Vd. Manhar Prajapati: First judge the symptoms of jirn ahar lakshana....utsah udagarsuddhi vego nu smayk rite avavu..laguta khut pipasa. You feel energetic without belching your natural vega stool urine are normal lightness feel hungry  thirsty these are symptoms for your food disgested. In ajirn indigestion you won’t get above symptoms. Now a days you most find out loss of appetite and heaviness of body and constipation...
Nitin Shah: Ajirne bhesajam wari
Dr. Dhara Barai: According astang harday in laghna 4 types sudhi also so sadhovaman and sadhovirechan is type of  laghan
Ayulink: Ajirna is Sanskrit terminology to describe digestive disorder, very broadly we can say it is indigestion. This is first phase of metabolism errors, from here it starts aama production or any disease formation. Agni disturbance is primarily connected with Ajirna.

Next topic for discussion: Sneha dravya - Theraputic uses
Vd. Jignesh Thakkar: Sneha dravya as said by charaka is  "स्नेहनम स्नेह विश्यन्द मार्दव क्लेद कारकम" Means drug which provides oleation, liquifies dosha, brings softness and bring kleda (?mosturisaton or liquification ) is called  snehan drug. So we should use snehana drug for these purpose. Sneha dravyas has three main indications 1) shaman sneha-pacifys doshas 2)sodhan sneha-liquify doshas 3) bruhan sneha-- increases dhatus. According to this three purpose we all  are choosing and using sneha dravyas. Shaman sneha is usually given as per this text" शमनो क्षुधतो अनअन्नो मध्य मात्र च सश्यते। Shaman sneh usually given before food and when patient is hungry. It is given in medium dose.
Shail H. Bhavsar: Sarpi, Taila, Vasa, Majja Are 4 main snehas. Among them ghrit is best b'cz of sanskar-anuvartan guna.

Vd. सर्पि is Best as pitta samak as per order Sarpi > majja> vasa> tailam For pitta saman and for vat samak  Sarpi < majja<vasa< tailam
Ayulink: For past 70 years, US FDA mentioned Sneha (All kinds od Fat) in restricted items list for health aspect. Fat is increasing cholesterol. Fat is responsible for cardiac disorders etc....Now the same FDA has reversed its actions. Now Fat is not in restricted list. They declared that there is no cholesterol, no good cholesterol, no bad cholesterol. Now american heart specialists say that Fat or cholesterol is not any more responsible for heart attacks and other cardiac diseases.
Ayurveda has clear view about Fats and its role in body maintenance. Ayurveda has classified sneh in three categories according to actions. Fat can be used for pacification of symptoms, it can be used for nourishment in the body and also can be used for detoxification process.
There is a recent trend in west about LCHF diet (Low Calorie High Fat) diet to loose weight. This is again very difficult to understand. People who follows this diet plan, they dont take wheat, rice or grains...they just take vegetables and fruits and some milk products, (if the person is not vegan). But on the other hand they consume lot of Fat....ghee, cheese, paneer, peanut, almonds etc.....and surprisingly they loose weight. I have few patients who follow such diet prog. I cant understand this. How come high intake of fat can help to reduce weight?
Vd. Sachin Kadlag: During snehapaana also, when patient takes only ghee in morning and meal only at dinner for 5 to 7 days, follows all rules of snehapaana parihaar, in most patients  2 to 4 kg weight is lost .
Ayulink: How it is possible, as we have learner that samanam Guna vardhanam. Means fat will increase fat (meda dhatu) in the body.
Croatia Branko Markovic: Noticed the same thing during sneha pana even 3 days will put of up to 7 kg from my experience
Vd. Sachin Kadlag: This is possible only when snehapaana parihaar is followed. Here we should think about poshana of saara meda dhatu , and nullification of  asaara meda . The ayurvedic logical theory behind this seems to be koshtha-shaakha gati .
Slovenia Anoop S. Anand: I think the logic lies in the fact that body will absorb the required amount of fat and the rest will line up the lumen of g.i tract and quantity above that fat will be excreted out as such..Just as we see in samyak snigdh lakshana during ghritpana..ghrita comes out through stools..Same way pure fat (oil/ghee) will come out through stools...also when excess fat is consumed the food passes through g.i tract faster (in fact slides through g.i tract)..Leading to lesser absorption and excretion of partially digested food..Hence the weight loss
Vd. Jignesh Thakkar: It depends on the matra of snehana as I discussed before
Slovenia Anoop S. Anand: As i said like samyak snigdha
Dr. R Prathiban: If we take carbohydrate ultimately it has to be converted into glucose for absorption of glucose insulin is needed or if excess glucose is taken it is stored as glycogen if it is unutilized will be converted into fatty acids, if we take high fat and low carbohydrate there is no need of insulin for its absorption, body makes glucose from fatty acids so production of fat is reduced in body. In LCHF diet also HDL will increase. We select dose for sneha pana but for LCHF diet dose is not selected, it is there daily food, it is digested and absorbed, they take only high fat foods, if everything comes out means how they survive. Is there any standardized method to calculate snehapana dose on the basis of kostha for shodana procedures? And how dose for shamana sneha is calculated?
Dr. Madhuri Patil Chaudhari: Jaaranshakti anuman for deciding the dose of snehpaan, more Jaaranshakti means more Agni , more Sneh but upto maximum dose or Uttam Matra of Sneh, otherwise more Sneh will increase Agni..leading to Bhasmak avastha.
Dr. R Prathiban: Standardized protocols needed nowadays, how to quantify agni and dose in milliliters
Dr. Madhuri Patil Chaudhari: Sneh here is assumed Cows Ghee. For other Sneha's, only  on the basis of Jaaranshakti. or rate and quality of digestiive power. I think Vd. Vasant Patil has done for Koshtha on the basis of Agni. The same method can b used to objectify the aagni n dose of Sneh.
Dr. R Prathiban: Before starting snehapana usually subject is prior administer with deepana and pachana so agni is corrected. Then why we have to consider agni as a dose deciding criteria
Dr. Madhuri Patil Chaudhari: https://googleweblight.com/i?u=https://www.researchgate.net/project/Standardization-of-Panchakarma-Procedures&hl=en-IN&tg=955&tk=4806922636401895738
Dr. R Prathiban: Consider mridu, madhyama and krura kostha what will be the dose of cows ghee in these kosthas?
Dr. Madhuri Patil Chaudhari: No if Aamavastha, pramathi dravyas like Mari sunthi are used for snehpaan. The existence of diseases itself  means Agni is not Samyak.
Dr. R Prathiban: If agni is not proper we can't give sneha
Dr. Madhuri Patil Chaudhari: Krur koshthi will need more than mrudu as Samyak Sneh lakshanas will need to be seen and dose of Sneh will have to be adjusted as per that. Still Cows Ghee can be given as it is Agnivardhak. siddha ghrut.
Dr. R Prathiban: Charaka mentioned about "hrisiyasi matra" how to decide hrisiyasi matra what is this in milliliters. Practically in all diseases we are not doing shodana and not in all avastha of disease shodana is advised, if we plan shodana with shenana as purva karma we have to correct agni
Dr. Madhuri Patil Chaudhari: 30ml is accepted popularly as Hrusiyaasi maatra.  On the basis of How much measure of Sneh, a person on an average,  can easily digest. Not exactly correct Agni. But Protect Agni. Because Shodhana Karma too cause Agnimaandya
Dr. R Prathiban: Who decided it as 30ml is it arbitrary value or any evidence based selection. For that we do paschat karma
Dr. Madhuri Patil Chaudhari: Please see this. Ongoing project Standardization. Everyone does as per his or her understanding of Agni.and Maatra.
Dr. R Prathiban: I agree we assume ourselfs and we do what we like to do. But things has to approached scientifically
Dr. Supratim Bir: Agni is d decider of snehamatra, there is no alternative that's for sure. Actually we are addicted to modern protocols, which count on formula considering a constant. But I do believe every individual is unique with their biological parameters. Modern protocols, particularly in case of medicine are loosing efficacy nowadays. Ayurveda is highest level of personalized medical science, can't be generalized.
Dr. R Prathiban: Let me elaborate my question on the basis of agni and kostha 1. Let as assume three 3 individuals with manda agni with Madhya kostha theekshna agni with mridu kostha, vishama agni with krura kostha. How you are going to decide sneha matra for samana sneha and shodana sneha.  2. Whether hrisiyasi matra sneha should be same or different for above condition.  3. How to measure effect of hrisiyasi matra in above conditions
Dr. Supratim Bir: Now I will ask what is the condition u considered for snehapaan
Dr. R Prathiban: Condition is pakshagata and shodana planned is virechanam
Dr. Supratim Bir: So in this case why to go for snehapaan, we can plan snigdha virechan with castor oil itself. We can start over 10, 15 & 20ml of Gandharba hastadi castor oil initially for nitya virechan. As per above mentioned kostha Or we can plan any of the  asthapan vasti, so that we can plan better as per kostha. My request to forum to consider my words as tadvidyasambhasha, u r welcomes to correct me. I will feel privileged
Dr. Siva Kumar: Can Fruits taken during snehapana?
Dr. R Prathiban: Im trying to discuss about poorva karma with sneha pana followed by shodana.
Vd. Pritam Bharat Veer:  Sneha Kalpana is a special mixture of ghee and other medicinal drugs. Ghee has several medical properties which can help increase the element of Agni without causing imbalance in Pitta. It increase the Ojas or the essence of energy. Especially suitable for Vata related ailments, Sneha Kalpana is sucessfully used for treating various disorders of the nerves and the mind, skin conditions etc.
Achapana Snehapana- Vicharana Snehapana is the consumption of Sneha (ghee), non-medicated or medicated form. This is not mixed with any other medical preparation or food items. It is used for lubricating and softening the tissues of the human body before carrying out the Sodhana therapy of Panchakarma. Vicharana Snehapana is the consumption of oil with other Ayurvedic medicines and other food products. It is also used for the lubrication purposes of the various parts of the human body. This can move through the rectal pathway, known as the Vasthi or the nasal pathway, known as the Nasya. External Oleation This process is also known as the Bahya Sneha. Externally, the treatment is carried out in the form of Siro Abyangam or Head massage, oil pulling or gargles, Abhyangam or Oil Massage and Lepa. I use Siddha ghrut in my daily practice. As I found a very good results.I use  panchatikta ghrut, Mahatiktak ghrut, Guggultiktak ghrut, Mahaklyanak, Kalyanak, Sukumarak ghrut etc. Ghrut kalpna prepared by Kerala company give nice results. I rarely use only Sneha  ,I use Siddha Sneha  , also sidhha teil for external use also nasya..
Dr. Piyush Dodiwadiya: In case of trauma The orthopedic surgeon says one should not do any massage over injured part, but ayurveda says do snehan (external)  Practically i have seen if we do massage over injured area then swelling markedly increased But the result due to massage with ayurvedic tail or ghee is i don't know
Vd. Pritam Bharat Veer:  Yes I also have seen very good results of bahya Sneha , murivenna tail dhara. HV seen re- union of two ends.
Ayulink: Gandh tail in fractures gives faster healing
Dr. Sushant Patil: Aabha and Lakshadi Guggul also have faster healing results. Use Of Sandhaniya Gana is also beneficial
Ayulink: Basically Sneha is used for building all dhatu.  What I believe.
Gary Yuen: Fat helps deliver nutrients to various tissues, per formulas, and included with food, so in itself it is not harmful. But certain fats such as in the form of regular use of oil instead of whole fats (sesame seed, olive, shredded coconut, nuts, etc.) has been shown to be harmful. The FDA, as well as the whole of medical studies, is influenced by industry, and it is difficult to trust general guidelines. As recently mentioned, many general testing procedures have been shown to be unbeneficial or harmful, for the same reasons.
Ayulink: What abt yamak Sneh? Mixture of two diff kind of sneha, e.g oil + ghee, or ghee + majja.
Gary Yuen: Of the handful of Western doctors that can be trusted, it's generally said that total cholesterol below 150 makes one free from risk of heart attack. I do not know about ghee or oil for therapeutic use. For vata, perhaps. Ghee is mentioned in earlier pre-ayurvedic texts such as the Upanishads though that could be a later interpolation. Ancient use of oil or ghee is unknown. According to early archaeological evidence, I think atasi (linseed oil) has been found. Ghee, it may have been according to season, considering the condition of the cow, breed, place, moment, etc. According to the Mahabharata, consumption of milk and ghee as it is done today, mass produced, would be adharm. It would also go against strict Jain rules for preparation. Ancient diet included many much healthier wild plants that few now eat (mustard leaves, amaranthus, chenopodium, flowers, etc.) that may have somewhat counteracted possible harmful effects of oil/ghee. Mass produced ghee is also quite different than made according to strict classical rules.I think amalaki has been shown to have powerful anti-diabetic and cholesterol lowering effects. Wild plants, fruits or vegetables, that we no longer eat, likely had higher anti-oxidant effects compared to modern foods or varieties of radish, eggplant, okra, etc. Dried amalaki has hundreds of times the antioxidants compared to blueberres and other common fruits. Between that and common foods, there's a wide range of nutrients and antioxidants that those living in cities no longer use. Highest known Vitamin C levels I've found are 5000 mg per 100 g, in a few fruits such as an australian variety of phyllanthus, relative of amalaki. 100x that of oranges. Between the highest and lowest, there's a wide range throughout nature. Various other foods such as raktasali are also no longer commonly eaten.Few people live past 100 w/o need for a doctor. Because of that, I think much needs to be questioned if one wants to attain what should be easily possible. I'm not sure if many well known ancient doctors or brahmins achieved as such. It's said Ramanuja might have lived to 120. As North India faced more invasion and other factors that changed dietary habits, perhaps more traditional habits can be found in Tamil Siddha texts.
Vd. Sureshbhai Chavda: Also given asthi shrunkhala ghrut in internal sneha
Shail H. Bhavsar: What is avpeedak sneha? What are the indications?
Vd. Pritam Bharat Veer:  Can you tell the dose and also of Gandh teil ?
Dr Piyushbhai Prajapati: 10 Drops in one cup of milk..acc.to me.. Sometimes in dry coughing; i used to give any mixture formulation by mixing cow ghee and til oil..
Vd. Pritam Bharat Veer:  I am also using Gandha teil in mouth ulcers. Good results. Also used for my kid in Acute Tonsillitis. Can you tell abt Asthi shrunkhala teil ??
Vd. Sachin Kadlag: I give 6 drops of Gandha taila in milk twice a day in adult and specially empty stomach in morning around  7 am and in evening around 5 to 6 pm
Shri Minoo parabia: Sold under the name, Spondygo is useful in Spondylitis and bone fracture healing.
Ayulink: Anybody has any comment or input for the use of majja?
Vd. Sureshbhai Chavda: Aja majja sidhh oil use in muscular distropy.
Gary Yuen: any opinions about atasi? Among some modern doctors, tila and atasi, as well as walnuts, are ranked among the best seeds and nuts, though with atasi being better. Other names of atasi include haimavati and uma, perhaps relating to Parvati?
Vd. Pritam Bharat Veer:  Ajay majja for basti , for  degenerative changes in joints
Vd Rameshwar Dhanaji: how to prepare majja sidha oil and its dosage?
Dr. Vivek D. Shah: Yes.it is very helpful.one of my friend was doing thesis work on majja basti & got excellant result in parkinsion and in शुक्रक्षय
Dr. Piyush Dodiwadiya: Most of the time Snehpan is very difficult to pt Before panchkarm we used it in very large dose Can we use ryles tube for that?
Dr. Mrugeshkumar: Acharya says direct  senhpan , recent we use any practical method for snehpan..? please guide..
Vd. Pritam Bharat Veer:  Sir it's very funny question but no we can't do
Dr. Piyush Dodiwadiya:Sir Practically I am asking With RT its very easy Why we can't do any specific reason?
Vd. Pritam Bharat Veer:  No specific reason but RT will become very painful for patients rather than having 30-60-90 ml ghrut in seconds. Instead we can advise to drink at once directly in throat.
Dr Piyushbhai Prajapati: What is role of achhapey ?
Dr. Mrugeshkumar Patel: can we use cap for sneh pan?!
Dr Piyushbhai Prajapati: No.. its waste idea.. for snehpaan of 50ml;pt has to take 20 caps
Gary Yuen: If it's not generally known, as our world is polluted, bone and majja has been found to have high levels of lead. It accumulates in bone, including ours, and other pollutants accumulate in fat. Perhaps majja should be limited to cases where there is no other option, or one perhaps should follow if possible using majja from wild forest animals such as those listed among those to keep in hospitals per classical texts. As we are carriers of hundreds of environmental pollutants, it may be advised to regularly fast and strictly follow diet to minimize such accumulation. Keeping body fat levels at only what is necessary may be best. We no longer go through periods of being without food so it is not needed to store energy for winter or other times.
Ayulink: It is not a good idea. It will be more painful and inconvenient for patient to insert RT every morning.
Sir, this is also not practical approach. Usually the dose of ghee reaches up to 120-150 gram in last days. If one cap contains 500 mg ghee, patient will need to take 240-300 capsules.
Achchapey is plain sneha. Without any other herbs or other ingredients.  Achchapey is also similar effective. But when u apply yr knowledge and add some more herbs according to patient's health condition (it is known as vicharana), it is more beneficial. I have used achchapey - plain ghee in many patients for snehpan for the purpose of Purva karma.
Dr Piyushbhai Prajapati: Sometimes i uses Achhapey as snehpaan.. and advise patient some medicine after snehpan with warm water.. For this patient can take easily sneh.. and then after because of  medicine tab with decoction etc as anupaan gives also good result to me.. so i use it.. Please comment on it sir..  For ex. Patient needed Mahatikta ghruta as sneh paan..  Then i use achhapey then after i advise patient to take 1 gm Aarogyavardhini tab with Tikt or mahatikt kasaay.. as anupaan.. Daily tab will be increased day by day.. 0.5 gm or as convenient..
Ayulink: For me this is ok. Achchapey can be used for shaman and bruhan also.
Dr. Jyoti Ashok: Certain kashaya is v. difficult for the pt. to swallow. What i do is add milk & little sugar & give like milk shake. They will easily drink that.
Croatia Branko Markovic: Very good trick is to give a patient after big dose (immediately after swallowing it) of sneha slice of lemon or orange to take a “bite”. Juice from this will clear taste of ghee and next day there will not be a problem for taking bigger doses.
Dr. Jaya Sambhus: In Ayurveda pachan starts from mouth  so if u skip primary pachan , sneh will not reach in ras dhatu So no use of snehpan . If pt refused by mouth use sneh basti It works
Ayulink: My practical approach: I usually give few nice tasty amalaki candies to patient to chew at a time...and then immediately I asked them to drink ghee or sneha in whatever dose. It is very good for patient to pass through mouth and throat..because by chewing few candies of amalaki, patient is having a coating of that taste on his tongue, throat and the original taste of ghee etc is much lesser felt. :)
Croatia Branko Markovic: This one is good  can people take lemon to chew a bit before what do you think? When taking after it clear completely taste of ghee.
Dr. Jaya Sambhus: Will try this. I used havaman harde tab n ginger pachak. It help to give good mouth test n smell with sneh pachan.
Dr. V B. Maheshwari: Approach is very good sir, with amalaki it will good for pachan of ghruta also. Harad is also very good option. In my opd its very difficult to conveyance the patients for sneha pana. But it will definitely work.
Croatia Branko Markovic: Here people don’t have a problem with taking ghee for sneha pana. even they come back all the time..only last dose when they are full is a bit problem..
Dr. Piyush Dodiwadiya: Some people used chocolate also
Dr. Samir Suneri: I using girta as a sneh dravya And using in case of constipation of old age patient. Started from 20ml per day in morning and daily I increase 10 ml for 5 to 6 days and I achieved good results
Canada Nitin Shah: Sneha dravya should be used externally and internally both and not just through the upper end of the GI Tract but also lower end of the GI tract too and from the other orifices too. See skin is the largest organ of the body and in Ayurveda it has been explained as seven layers and we can consider these 7 layers in relationship with 7 vital tissues
Mona Patel Naturopath: Four types of Snehapana dosage schedules  are indicated- Pravara Matra-Sneha digest in 24 hrs.Madhyama Matra- Sneha digest in 12 hours.Avara Matra - digest in 6hrs.Hrasiyasi Matra with arohana krama-digest in minutesThese to be fixed, keeping in view the various factors like state of agni, clinical condition,Prakruti of patient, etc.and aim of snehapana.
