Ayulink: There is no possibility of Kandu (itching) without involvement of  Kapha, that is what ayurveda texts says. In winter, we see many patients, suffering with itching in dry skin (Vata major condition). Are they both different conditions?  Should there be different line of action?

Dr. Sushant Patil: Here one of my patient having Itching I had taken full case. But I found no involment of  Vata. So I consider his job n work. So, I count use of soap bar again and again is also a cause.

Ayulink: We have many cases like this. Which dosha will be considered for the treatment? Kapha or Vata ? Most of them don’t  show any signs of Kapha aggravation, and still they get lot of itching,  use of soap can increase more dryness, that is my opinion.

Dr. Piyush Dodiwadiya: Itching is a symptom of certain disease Primarily should think about provisional diagnosis. Mostly allergic or fungal or due to some inflammation. Nonspecific itching may some contact dermatitis. Or prickly heat or due to excessive sweating. Medical condition such as increased bilirubin level can cause itching.

Vd. Pritam Bharat Veer: If you take total history somewhere we can fine dosh drushti. I have one pt. with itching over the elbows which is increased in this season also one more thing that she has jirn pratishyay along with that the area has become blackish. So according to me dosh dushti is Vat , Kafa and Rakta.  Itching can also be due to Shitpitta where there is involvement of pitta dosh. In kandu Rakta dushti is compulsory present. If we work on rakt dosh problem solved upto 50%.

Branko Markovic: In EU there is a lot of people with hands like this and also present with crack in. Usually there is no diagnosis from allopathy but if they don’t know what to do they say it is neuro- dermatitis and give corticosteroid cream. Which I found pure rakta dushti do to incompatible  and bad diet. In most cases like this of racking and itching line of treatment would be towards pitta and kapha medicines and pitta pacifying diet. it can bleed also or just be reddish.

Dr Nrupesh Gupta: Itching is the symptom which does not indicates only skin dis because even a constipated pt. also comes with the sym itching. Detail history will reflect the main cause of itching.

Dr. Ridhdhi Sardhara: In charak Samhita Kandu is karma of pitta & kapha dosha individual.

Vd. Jayesh Thakkar: When there is aama involvement indigestion or kapha overlapping agni, itching common perhaps kandu kapha refers this. Practically have found any deepan-anulomana medicine helping.

Dr. Ridhdhi Sardhara: Vikrut dosha karma,  According to Astang hriday & shusrut samhita  kandu is karma of swed vruddhi.

Ayulink: Scaling in Palm -  personally I consider it as sign of chest congestion. Kapha aggravation may shows such symptoms.

Dr Ashi Vaidhya: It's the allergic reactions of soap bar

Dr Jayesh Patel: Yes, rakt dushti along with kaph is may be main reason for itching

Dr. Piyush Dodiwadiya: Exfoliation of skin As a routine, just appy coconut oil to prevent dryness

Vd. Pritam Bharat Veer: For kandu I used to give dhavan with triphala kwath and nimb then local application of mahatiktak ghrut or ointment. It works. Simply i tell the pts to apply milk before going to bath. What others use for kandu as a disease?

Dr. Piyush Dodiwadiya: Is anyone diagnosed and treat scabies. In scabies itching is horrible.

Ayulink: No. I have not seen any Pt of scabies in my 30+ years of practice. But I have tried with many Pt with different kind of itching. Virechana,  takradhara and complete body lepan sometimes are my favorite. My choice of drug for itching is tab.  Gandhak rasayana.

Dr. Piyush Dodiwadiya: Scabies is usually found in poor socioeconomic people.I diagnosed many case by didn't try ayu medicine because lack of knowledge. In scabies now a days drug of choice is permethrin local application, it is toxic drug, which kill the organism. So is there any lepa or local application which does the same thing?

Ayulink: I have to check as I have no experience. Can u share few photos of scabies?

P A. Deshpande: Itching is effect of presence  of  toxic substance . Mosquito ir bugs bite results in itching due to penetration of toxic substance. The body mechanism neutralizes during which itching effect is exoerianced. Same thing but in reverse manner. The toxic substance  below the  last strata of skin is tried to be thrown  during  which scars, cuts or burns even effect is seen. When excess saturation  is the cases itching is non stop. Overlapping skin polyester innerwares socks create moisture due to setting. When low aeriation or oxygen supply  fungal infection take place causing itching  .  To cure. Sponging with hydrogen peroxide  ir triphala water followed by immediate application of coconut oil give instant relief.  For cure remove factors responsible by  using cottonware  loose cloth  periodical sponging and lubrication,  fasting ,  detoxification pranayam etc with duet changes.  Better be on fruits and veggies.

Dr. Piyush Dodiwadiya: Yes, I  get and post Download from net. Initially this kind of finding we may get. Itching during night hours is most. Over chest upper limb and inguinal region. Find irruption between fingers, web space. Patient makes scratches and then feel better.

Ayulink: One more question from other group : Panchtikta ghrut guggulu and Panchtikta ghrut, both are useful in skin problems and itching. In which condition ghrut alone should be given and on which condition ghrut with guggulu should be given?

Shri Minoo parabia: Scabies insect lay their eggs below the skin. When larvae comes out they start mining through the issue causing severe itching. Worst part is the patient can not really scratch the actual point as it is underneath the skin unless one bleeds! Out side application has to be real strong that can penetrate through the skin. Internal medicine has to be equally strong to get rid of larvae.

Vd. Jayesh Thakkar: Exactly its challenging to treat scabies. it spreads fast and needs immediate strong medicine. have got mix result. its quite common in humid region.

Shri Minoo parabia: Permethrin a white milky liquid was used as topical liniment. I believe, a prolonged bath with neem and panchvalk decoction may help. I'm not sure friends. I will welcome experts opinion.

Vd. Jayesh Thakkar: I use chalmogra (tuvrak) tail to apply before bath- tankan mix water for wash.some relief but temporary.

Shri Minoo parabia: Our bed sheets develop couple of million mites within a week, however clean it may appear. Through our perspiration and skin flakes we provide them nutrition. Our ancient habit was to keep the mattress and bed sheets under the sun was perfect. Even beds were taken off the ground, every day.

Dr. Vasupradha R: I guess the snigdhatwam of kapham s decreased due to the Vatha vrudhi in winter

Dr. Hiral Khakhar: In kaphaj kandu- avoid causative factor of kapha vitiation,use katu,tikta,kashay,ushna food and- medicine,langhan,exercise, avoid divaswap. In pittaj kandu- avoid causative factor of pitta vitiation, intake of medicated ghee,use madhur,tikta,kashay medicine n food, apply sandalwood oil,virechan. Vataj kandu- avoid causative factor of vat vitiation, administration of medicated oil ,ghee,massage..madhur,amla,lavan,ushna food and medicine. We should also think about worms and give krimihar treatment. Blood purifier like mahamnjishthadi kadha,khadirarista,neem,daruhaldi churna..give triphala and correct digestion..

Vd. Avni Kaneria: Itching due to rakta dosha ...in this condition Virechan karma/rakta-mokshan can help

Dr. Megha Patil: Sir wt  we will give for pt ithching over groin region

Ayulink: Usually I prescribe Cutis powder for local applications.

Dr. Jyoti Ashok: Cutis pdr relieves itching immediately. if the area is dry I advise panchavalkala ointment.

Dr. Vivek D. Shah: Oraly only gandhak rasayn can help in this condition...?? If go with the shaman chikitsa... Sir Hw abt Panchtiktaghrut guggulu. wid gandhak rasayn??

Ayulink: Yes. Orally we can give anything. Like manjistha, khadir, kaishor etc.

Dr Mario Herrera: How much time before we can see the effect of  Manjisrha or any other Ayurvedic herbs for skin problems?

Ayulink: I think this is not easy to predict. Buy in practice, I expect some improvements in 15 days, if my combination of medicine is good. Panchkarma is much faster. Within 15 days after virechana we can expect much more relief from the symptoms.

Ayulink : Panchtikta ghrut guggulu and Panchtikta ghrut, both are useful. But before that one must know the exact purpose of them. In which condition ghrut alone should be given and on which condition ghrut with guggulu should be given? Can anybody explain?

Dr. Jyoti Ashok: Guggulu usually increases dryness. So if the itching is due to dryness ghruta is good.

Dr. Vivek D. Shah: Don’t  know about particular authentic different uses but as per my interpretation gugglu can only suggest if meda dhatu/kaph dosha dominant skin problem n ghrit can give if vata dominancy.
