Next topic for discussion:  :  Management of liver disorders - e. g. Fatty liver, Jaundice, hepatitis etc.  by #Ayurveda (No theoretical details abt liver disorders please).

Dr.Surendra Davanagere: Arogyavardhani,  Bharat Bhaishajya Ratnakar 1/448
Arogyavardhani as the name indicates destroys all diseases & promotes Health. It is the  drug of choice in all skin diseases. Excellent in Pandu as well as enlargement of Liver & Spleen due to ingredients like Loha Bhasma, Tamra Bhasma & Kutaki. It also acts as Deepak, Pachak, Malashodhak, Hrudya & controls Mansa & Medavruddhi. Indications:  Skin diseases like Ekakushtha, Vicharchika, Vipadika, Shwitra, Enlargement of Liver & Spleen, Sthoulya, Jwara, Malabaddhata, Accumulation of Mala in Genitalia Special precaution -  Excessive motion, may cause weakness, bleeding with stools, Dhatukshaya

Dr. Prerak Shah: For non-Ayurveda friends: As such formulas like Arogyavardhini ras, contain heavy metal like Mercury, sulphur, copper and others - it should be consumed only under supervision of qualified Ayurveda doctor.  There may be many serious side effects of such formula.

Shri Minoo parabia: Yes. True. " sage who invented Arogyavardhini is worthy of Nobel prize". Thus spake shri Bapalal Vaidya during one of the conversations.
So, rider is, must have been made by trust worthy vaidya. Shat naman to the memory of my pratah smaraniya guruji.

Vd. Dhruti Kagrana: Bhumi-aamalaki  (Phyllanthus niruri ) is also a choice of drug for liver disorders..

Vd. Ketan bhimani: Best pitta virechak - kadu. kadu kariyatu in cap syrup n yavkut base also available.

Dr. Prerak Shah: So basically in liver disorder do we suggest pitta virechak first? Or diet control? Or any thing else? What can be the line of treatment?

Shri Minoo parabia: Diet control is the key in my personal view. I have seen ethnic healers using variety of materials to treat liver disorders.
Bhumianalki, Ghrit kumara, Indigo churna, Kukkadvel, Bitter bottle gourd….Drugs are administered orally or as nasal or ear drops. No scientific documentation exists. But I have seen long queues of patient awaiting treatment and the experience of people cured. This needs further research and documentation.

Dr. Archana Panchal: In diet stop ghee oil & all type of sticky items.citrus fruits consume more.

Shri Minoo parabia: Cow ghee is good and recommended.

Vd. Ketan bhimani: Bhojanagre Sada pathyam lavan aadrak bhakshkakam.

Dr. Prerak Shah: @Minoo sir, please let us know nasal and ear administration of medicine for liver disorder. This is very interesting.

Shri Minoo parabia: Kukkadvel, Luffa echinata fruits are soaked in water overnight and the filtrate is used as nasal drops. Irritation is abominable but jaundice is cured. A gentleman at surat uses herbal ear drops with success but doesn't reveal the name of herb. Though he learnt many things from me😄. Same excuse... guru agya.

Dr. Jaydeep: In query. Experts plz share their opinion.
In the management of kamla (Jaundies) , so many ghrita preparations have been described in classical texts ( charak , astang ridaya and many more).  As per pathophysiology of kamla e.g. hepatitis , all lipids are to be strictly avoided.
Wts the conclusion regarding this?

Shri Minoo parabia: .....and these preparation s do yield good results. They are anubhut.  Someone must compare this with latest debate on cholesterol. It is said that lower cholesterol suggest the impaired liver function. Perhaps the time is up to revisit and revise the understanding.  Ayurveda has time tasted interpretation s

Dr. Chirag Dobariya: My opinion - Mostly Ayurveda base kamla pathophysiology develops after pandu. In pandu rogi excessive done kamla निदान सेवन than kamla develops that time use ghee preparation and most thing is Jaundice is not a kamla but it is jwara.

Dr Dipak Shirude: Only practical things about liver disorders in the perspective of Ayurved : Liver is the popular area for Ayurved. Even modern highly educated consultants rely upon Ayurvedic liver preparations. ……..I have a good experience regarding liver disorders which were quite difficult prognosis. e.g liver cirrhosis with fatal complications, liver parenchymal disease, hepatitis A/B/E +ve, shrunken liver with advice of transplant, multiple hydatid cysts in liver, fatty liver and ca of liver etc………In these cases, except ca liver or metastasis in liver are totally incurable, in my experience. All the other situations can be tackled satisfactorily.
Some notable things :
1. We must find history of modern medicine taken by patient. Nowadays, our soft organs are loose target for modern medicines. Additional study of hepatotoxic drugs is essential for a vaidya, so that he can decide proper way of cure for his patient.
2. History of consumption of alcohol.
3. History of living with pet animals.
4. History of abnormal food habits.
5. History of sexual or blood contacts.
6. Amongst all the organs, liver is a organ which responds earlier to our i.e. ayurveda medicines.
7. We must review our chikitsa, if we do not get successful results within seven days. Count of bilirubin may be an exception to the above rule, yet the bilirubin count doesn't seem to be in control up to 15 days almost in some cases, the patient still clinically improves !!
8. Raw and fresh herbal medicines have no other better option for genuine result. A fresh aloe pulp or freshly prepared eclipta juice shows very sure and early results. The kalpas of these same medicines, although manufactured by reputed companies, are always behind in the context of fresh herb.
9. One must avoid abundant and complicated combinations of our medicines in liver patient. Single drug or single classical kalpa proves more suitable than multiple mixtures.
10. One can predict the status of liver physiology and doshik indulgence in liver by neat observation of patient's tongue and skin !! In some patients, unexpected blackening of scalp hair is seen after liver treatment. That is good side effect !!
11. Virechan is a great weapon in the hands of vaidyas against many liver ailments. Tricky use of stepwise recipes said in sansarjan krama also helpful in the treatment of liver diseases.
This is what I reminded and could put in front of you all regarding liver and Ayurved. Rest will be as opportunity comes again !!

Satyen: Does high cholesterol also suggest liver malfunction?

Dr Dipak Shirude: Sure

Dr. Pradip Mohan Sharma Jaipur: Fatty liver and my experience…….Fatty liver is propagated as incurable malady by modern medicine. But this is absolutely their big lie. They always think that only those diseases are curable what they can treat. A great humiliation against humanity. However I have treated fatty liver with this formulation: लौह भस्म 250 mg, रोहितक लौह 250 mg , शुध्द शिलाजीत 500 mg,
मूलिका क्षार 500 mg, ताम्र भस्म 60 mg, BD, पुनर्नवाष्टक क्वाथ
In case of jaundice I always prefer to prescribe following हिम
पिंड खजूर 2 नग, मंजीठ 2 ग्राम, सनाय 1 ग्राम BD
Only this formula bring bilirubin levels down within 2 couples of days, with this I never try other medicine, after 4 days I prescribe other medicines like रोहितक लौह, पुनर्नवा मण्डूर, मूलिका क्षार, आरोग्यवर्धनी वटी etc……..External use if GHEE for massage also produces good results but I have never seen internal use of this

Dr. Madhuri Patil Chaudhari: Avrodhjanya kamla or hepatitis, one ayurdoc i knew as child,  used to give Gorochana powder with curd...smtimes bile salts with curd..to reduce agnimandya-related Aadhman or Ajirna.  abdominal bloating , indigestion etc. (due to less bile in duodenum) reduced.

Dr. J. T. Contractor: Apart all the treatment of liver with due respect, not to forget ultraviolet rays blessed up on liver that's the nature, one cures animals in jungle, by instinct animal do take tender sun rays, for the liver to regenerate it's functionality, incubator system who ever has invented must b inspired by nature I suppose, comment welcomed on holistic remedy pl!

Dr. Meghdoot Satvekar: Kalmegh decoctions( extract of leaves / क्वाथ ) is a liver tonic. Many things affect the liver like too much alcohol consumption, powerful medications, eating foods sprayed with pesticide and consuming kalmegh is one of the best ways to keep the liver functioning properly. Kalmegh has got anti inflammatory properties making it ideal for treating arthritis and gout. A simple decoction made by boiling 15 to 20 leaves of fresh or dried kalmegh leaves in a cup of water till it is reduced in half can be used for treating liver diseases. The decoction is very very bitter and can be consumed along with  honey / sugar .

Dr Feroz Khan: Kasmard swaras with black piper is usefull in liver disease.

Vd. Kundan Gadhavi: in jaundice or hepatitis triphala kwath, guduchi svaras, daru haridra etc…….in obstructive jaundice tikshna ausadh like trikatu ,matulung rasa to be give………in kamla in acute phase mrudu ruksha virecha should be given like avipattikar while in chronic phase or to prevent recurrence can give snehapan then virechan

Dr. Pradip Mohan Sharma Jaipur: If physical obstruction like stone or fat, is there at CBD, only then this is obstructive, otherwise all icterious conditions are infectious.

Vd. Kundan Gadhavi: yes of course then only can use tikshna drug…..nasya is mentioned for kamla in chakradatt

Dr. Pradip Mohan Sharma Jaipur: Yes of course, but this is only to reduce the concentration of prakupit pitta from URDH-JATRUGAT pitta. This is not treatment as whole……….The हिम as i served in my early post, is also suggested by CHAKRADATT or may be SHARANGDHAR known as पित्त प्रमाथि हिम

Dr. Meghdoot Satvekar: Bhumi Amla…..In liver diseases …….Bhumi Amla is a medicinal plant used in India. It has several medicinal properties such as hepatoprotective, antiviral, anticancer, anti-diabetic, antibacterial and anti-inflammatory properties.
It cures inflammation of the liver. The extract of the plant is used to cure Hepatitis A, Hepatitis B, and Hepatitis C. It gives relief from jaundice, dyspepsia, ulcers, wounds, chronic dysentery, diabetes, dropsy and menorrhagia. Because of its bitter, cold and astringent nature. The juice of the leaves can be used for abdominal pain. The leaves crushed with milk can be taken for any kind of urinary disorder.  It lowers the lipid activity. It can be used for any kind of liver problem and clearing gall bladder stones. Leaves of the plant contain alkaloid phyllanthoside with strong antispasmodic activity and helps in treating spasms.Bhumi Amla shows anti-viral and anti-microbial effect. The juice of the plant is used to cure liver disorder. It can be used as a drink during dysentery, to stop diarrhea, vomiting stomachache and constipation.

Add Vitro where biliary canaliculi is involved. ………..Fatty liver:- Katuki, Amalaki, Barley diet, Guda haritaki(jaggery with chebulic myrobalan)…………Wilson disease:- Gandhaka(sulphar) rasayana, Punarnavadi kwath, pippali………..Obstructive jaundice:- Citron(bijora) with trikatu
……….Hepatitis C:- Bhumi amalaki, Sharpunkho(tephrosia purpura), Kakamachi(solanum nigrum), Rohitak(tecomella undulata).

Dr. Prerak Shah: So basically in liver disorder do we suggest pitta virechak first? Or diet control? Or any thing else? What can be the line of treatment?

Vd. Pankaj Chhayani: Virechana is prime treatment in all type of udar roga, but have to see bala of patient...Virechana with Katuki(picrorhiza curroa) is best choice, Snuhi (common milk hage), haritaki(chebulic myrobalan)are also good...
Milk diet is preferable where ascites is present...

Vd. Kinjal Patel: Snehapan contraindicated in udarroga....classical virechana can we do ?? Charak only indicate nityavirechana

Vd. Pankaj Chhayani: Nitya virechana is not possible ,so we do virechana as per pts strength..

Vd. Kinjal Patel: But snehapan ayogya udar rogi

Vd. Pankaj Chhayani: There is no need of snehapana, I heard one case in which pt died because of snehapana..charaka has denied snehapana in condition of Sama  pitta... Due to snehapana , pt will go in coma as charaka has mentioned in sutra 13........Not only in Liver disease , in paralysisi  also snehapana should not be given where Pitta is in  Sama  condition..

Vd. Pulkit D. Patel: aavrut and aavaran chikitsa can also useful.

Vd. Kinjal Patel: Can we given direct sodhan dravya without snehan and swedan?

Vd. Pankaj Chhayani: Yes, why not..snehapana is for utklesha for dosha.. Where dosha is already in condition of utklesha, there is no need of snehapana.
